Intake Assessment

Our intake Assessment is a comprehensive yet compassionate evaluation designed to understand the unique needs of each patient. During the assessment, our team gathers detailed information about the patient’s medical history, daily routines, and personal preferences, as well as assesses the home environment to ensure safety and accessibility. This initial visit sets the stage for a personalized care plan, ensuring that our services are tailored to promote independence and well-being.

Level 1 In-Home Care

Level 1 home healthcare is designed for individuals who are largely independent but require minimal assistance to safely remain at home. It typically includes basic support such as help with personal hygiene, light housekeeping, meal preparation, and occasional transportation for appointments. The goal is to offer just enough care to maintain safety and quality of life without the need for intensive medical supervision, with services usually provided on a once- or twice- weekly basis to supplement the person’s own abilities.

Level 2 In- Home Care

Level 2 home healthcare is for individuals who require more frequent assistance than Level 1 but do not need around-the-clock care. It typically includes support with daily activities such as bathing, dressing, medication management, light housekeeping, and meal preparation. Services may be provided several times a week, depending on the individual’s needs. Level 2 care often involves more hands-on help with mobility or assistance with chronic condition management, but the individual can still generally remain independent with some supervision and help from caregivers.

Level 3 In- Home Care

Level 3 home care is designed for individual’s with intermediate care needs who require more frequent and comprehensive support than lower care levels provide. This level of care typically includes intensive personal care such as assistance with bathing, dressing, toileting, and mobility, combined with regular visits from skilled nursing professionals for health monitoring and medication management. In addition, Level 3 care often incorporates allied health services like physiotherapy or occupational therapy to help maintain or improve functional abilities, ensuring that individuals can safely continue to live at home with a structured support system in place including around the clock care and skilled nursing care combined.

My name is Jordan, and I’m the proud owner of Highlands Best In-Home Care. I’d like to take a moment to introduce myself and share a bit about who we are and what we do.

I serve as the Intake Coordinator, specializing in in-home healthcare coordination and client onboarding. With over seven years of hands-on experience in home care, I made the decision in March of 2023, to launch my own agency. Since then, I’ve expanded my certifications to better meet the diverse and evolving needs of our clients and their families.

At Highlands Best In-Home Care, I personally visit each client’s home to conduct a thorough assessment alongside their family. This allows us to create a customized care plan tailored specifically to the patient’s needs. I truly have a passion for what I do and feel honored to support the families we serve. We are now pleased to accept private insurance, Medicare, and Medicaid, enabling us to assist a wider range of individuals and provide more comprehensive support.
