Intake Assessment
Our in-home assessment is the first step in creating a personalized care plan tailored to each client’s unique needs. During this visit, we take the time to get to know the client, understand their daily routine, and identify the level of support required to ensure their safety, comfort, and overall well-being. This process allows us to provide care that is both effective and thoughtfully aligned with each individual.
Level 1 In-Home Care
Level 1 care is ideal for clients who are mostly independent but benefit from supervision, companionship, and light assistance throughout the day. This level focuses on maintaining routine, safety, and emotional well-being while allowing clients to remain as independent as possible. Services may include companionship, light housekeeping, meal preparation, medication reminders, and transportation to appointments or outings.
Level 2 In- Home Care
Level 2 is designed for clients who require hands-on assistance with daily living activities and ongoing support to ensure safety and comfort. This level includes help with bathing, grooming, toileting, mobility, and meal assistance, along with medication reminders and supervision. Clients at this level may also have mild to moderate cognitive decline and benefit from structured routines and consistent caregiver support.
Each client is carefully assessed and placed into the appropriate level of care to ensure they receive the right support for their individual needs.
Level 3 In- Home Care
Level 3 care is intended for clients who require extensive, hands-on assistance and continuous supervision due to higher physical or cognitive needs. This level includes full support with activities of daily living, transfers, incontinence care, repositioning, and advanced dementia or Alzheimer’s care. Care at this level prioritizes safety, comfort, and close monitoring to ensure the highest quality of support at all times.

Jordan Crabtree is the Owner and Founder of Highlands Best In-Home Care, where she brings over 8 years of experience in the in-home care industry, along with a deep passion for providing compassionate, personalized support to families in need. With a strong background in both hands-on caregiving and care coordination, Jordan has built her agency around one core mission: to help clients remain safe, comfortable, and cared for in the place they call home.
Throughout her career, Jordan has worked closely with a wide range of clients, including those living with dementia and Alzheimer’s disease, individuals requiring assistance with daily living activities, and those needing ongoing companionship and support. Her experience has given her a strong understanding of not only the physical aspects of care, but also the emotional and psychological needs of both clients and their families.
As the owner, Jordan is actively involved in every aspect of the business, ensuring that each client receives individualized attention and a care plan tailored to their specific needs. She conducts in-home assessments, collaborates with families to develop personalized care strategies, and works closely with her team to ensure consistency and quality in every interaction. Her hands-on approach allows her to maintain a high standard of care while building meaningful relationships with the families she serves.

She is currently pursuing her Clinical Medical Assistant and Administrative Medical Assistant credentials, further strengthening her knowledge and ability to support clients and families with a well-rounded, informed approach to care.

Jennifer Holleman serves as the Manager at Highlands Best In-Home Care and brings over 15 years of extensive experience in the healthcare field. As a Certified Nursing Assistant II (CNA II), Jennifer has built a strong foundation in both hands-on clinical care and the operational leadership required to ensure high-quality, consistent in-home services for every client we serve.
Throughout her career, Jennifer has worked closely with a wide range of patients, including those requiring memory care, hospice support, and long-term in-home assistance. Her experience in dementia and Alzheimer’s care allows her to approach each client with patience, understanding, and a deep respect for their individuality and dignity. She is highly skilled in recognizing subtle changes in condition, implementing appropriate care strategies, and ensuring that each client remains safe, comfortable, and supported in their home environment.
In her role as Manager, Jennifer oversees the day-to-day operations of care services, ensuring that every client receives personalized, reliable, and compassionate care. She plays a key role in caregiver coordination and scheduling, staff training and support, client care plan implementation, and ongoing quality assurance. Jennifer works closely with families to ensure clear communication, consistency in care, and peace of mind knowing their loved one is in capable hands.
Jennifer is also deeply involved in onboarding and mentoring caregivers, helping to maintain the high standards that Highlands Best In-Home Care is known for. She ensures that all team members are properly trained, supported, and aligned with our mission of providing care that goes above and beyond expectations. Her leadership helps create a strong, dependable team that families can trust.
